Little aged 41 years a resident of Bellaire county Belmont State Ohio who being duly sworn according to law, declares that he is a pensioner of the United States, enrolled at Columbus Pension Agency at the rate of 24- dollars per month, certificate no 19.911 by reason of disability from Gun Shot wound in right elbow joint icurred in the Mil. service of the United States while serving as a private in Co 'B' 12" Ohio Vol. Cavy.

He also applies on account of Gun Shot wound in right side affecting kidneys.

that he hereby appoints with full power of substitution and revocation his true and lawful attourney to prosecute his claim.
