Will you please examine the Medical Records in your office, and report for what disease or injury William Little late Pvt Co B 12' Regiment, Ohio Cavl was treated while in he claims shot by accident while at Camp Chase Ohio as a paroled prisoner on the 29 of May 1865.

in the line of duty, at or near a place called Camp Chase, in the State of Ohio on the 29th day of May AD 1865 he:

(was accidentally shot by one of the Guards belonging to 88th Ohio OVI. The said applicant being a paroled prisoner. The ball entering the right arm at the elbow shattering both bones and passing hence through to the right sille (?) arm. Lodging in the back bone. The arm receiving such an injury as to permanently disable it.
